FEES and INSURANCE
Dr. Carolyn Stack treats mostly self-pay patients, although she does accept insurance plans with policies that provide out-of-network benefits. Please note that that Dr. Stack is not a provider for any HMO products including Medicare. It is the patient's responsibility to ensure that he/she is entitled to reimbursement from his/her specific insurance policy and to pay for services rendered.
Patients using Blue Cross/Blue Shield plans must submit claims directly (using an invoice provided by Dr. Stack) and are responsible for the full fee when the invoice is provided. Patients using other out-of-network companies are responsible for co-pays and deductibles based on their individual policy.
Dr. Stack accepts checks, cash and certain credit cards (Visa, MasterCard, and American Express). A 3% service charge is included on credit cards. Please note that payment is expected at the time of service or within 14 days of receipt of an invoice.
Appointment times are reserved in advance. A minimum of two business days (48 hours)
notice for cancellations is required. Appointments that are missed without 48 hours notice will be charged at the full session rate.
Plan to arrive at least 5-10 minutes prior to an initial appointment time to complete necessary paperwork. This includes the following: signed and initialed HIPAA form; new patient information form; information about psychological and medical history including a list of current psychotropic medications.
Dr. Stack will also email these forms (and directions to her Cambridge office) to prospective clients prior to the appointment.
Dr. Stack complies with the Privacy Rules established under HIPAA, the Health Insurance Portability and Accountability Act. The Notice of Privacy Practices describes the circumstances under which psychologists are mandated or allowed to use or disclose psychiatric information. This notice is given to all new patients, and a copy is available upon request.
CONTACT OUT OF SESSION
For scheduling changes Dr. Stack is available by email or in person. Telephone messages and emails will be returned within 24 hours on business days and before Monday on weekends. For urgent matters Dr. Stack is available by phone or email. She returns messages as soon as possible but may not be able to respond immediately. Patients experiencing an emergency who are unable to reach Dr. Stack immediately are instructed to call 911 or to go to the nearest emergency room.
